KOMPLIKASI VENTRIKULOPERITONEAL SHUNT PADA PASIEN PEDIATRIK USIA 0-2 TAHUN DI RUMAH SAKIT UMUM PUSAT NASIONAL DR CIPTOMANGUNKUSUMO

VENTRICULOPERITONEAL SHUNT COMPLICATIONS ON PEDIATRIC NEUROSURGERY PATIENTS 0-2 YEARS OF AGE AT CIPTOMANGUNKUSUMO HOSPITAL
ABSTRACT
Introduction: Ventriculoperitoneal shunt is the procedure of choice in the management of hydrocephalus. However, the ventriculoperitoneal shunt procedure also carries a risk of complications. In Indonesia, data on the incidence and prevalence of ventriculoperitoneal shunt complications have not been obtained.
Aim: In this study, we examined the complications of shunts and the factors associated with the duration of time
Methods: This study is a retrospective study of pediatric patients aged 0-2 years with hydrocephalus who underwent ventriculoperitoneal shunt surgery for the first time at Cipto Mangunkusumo Hospital in 2015-2020 and experienced complications. The duration of time the complications occurred were divided into before and after 6 months. Demographic data, clinical data, and types of complications were analyzed descriptively
Result: From January 2015 to March 2020 at Cipto Mangunkusumo Hospitalthe , 168 ventriculoperitoneal shunt operations were performed at the age of 0-2 years. It was found that 20 patients (12%) experienced complications with a duration of less than 6 months by 7.8% and more than 6 months by 4.2%. The total complications that occurred were mechanical complications (7.2%) , infection (3.6%), mechanical complications (7.2%) and over drainage (1.2%).
Discussion: The percentage of complications of ventriculoperitoneal shunt in patients aged 0-2 years at Cipto Mangunkusumo Hospital, in the first 6 months was 7.8%, with the highest incidence wasof infection (3.6%) which was lower than the literature, while the percentage of complications of ventriculoperitoneal shunts which was more than 6 months . was 4.2% and mostly caused by distal malfunction (3%).

ABSTRAK
Pendahuluan: Ventrikuloperitoneal shunt merupakan prosedur terpilih dalam tatalaksana hidrosefalus. Namun demikian, prosedur ventrikuloperitoneal shunt juga memiliki risiko yang disebut sebagai komplikasi. Di Indonesia belum didapatkan data estimasi insiden dan prevalensi komplikasi ventrikuloperitoneal shunt.
Tujuan: Dalam penelitian ini, kami meneliti tentang komplikasi shunt dan faktor-faktor yang berhubungan sesuai dengan durasi waktu.
Metode: Penelitian ini merupakan penelitian retrospektif pasien pediatrik usia 0-2 tahun dengan hidrosefalus yang dilakukan operasi ventrikuloperitoneal shunt pertama kali di RSUPN Dr. Cipto Mangunkusumo (RSCM) pada tahun 2015- 2020 dan mengalami komplikasi. Durasi waktu terjadinya komplikasi dibagi menjadi sebelum dan sesudah 6 bulan. Data demografi, data klinis, dan jenis komplikasi kemudian dianalisis secara deskriptif.
Hasil: Pada periode Januari 2015 hingga Maret 2020 di RSCM telah dilakukan total operasi ventrikuloperitoneal shunt sebanyak 168 operasi pada usia 0-2 tahun. Didapatkan 20 pasien (12%) mengalami komplikasi dengan durasi kurang dari 6 bulan sebesar 7,8% dan lebih dari 6 bulan sebesar 4,2%. Persentase kKomplikasi total yang terjadi adalah infeksi (3,6%), komplikasi mekanis (7,2%) dan over drainase (1,2%).
Diskusi: Persentase komplikasi ventrikuloperitoneal shunt pada pasien usia 0-2 tahun di RSCM dalam 6 bulan pertama adalah sebesar 7,8%, dengan insiden terbanyak adalah infeksi (3,6%) yang lebih rendah dibandingkan literatur, sedangkan persentase komplikasi ventrikuloperitoneal shunt lebih dari 6 bulan adalah 4,2% dan terbanyak disebabkan oleh malfungsi distal (3%).
